Let nobody’s mother die of tuberculosis in the hospital where my mother got treatment

Decades ago, an old lady got admission in a private room of Gulab Devi Chest Hospital. She was suffering from tuberculosis .She was treated well. When she got better, her son visited the Medical Superintendent and told him that he wanted to construct a ward in the Hospital for poor patients. The Medical Superintendent gladly agreed when the donor said, ” My mother was treated in your Hospital and she got better .Fortunately , I could afford the expenses but when I see many people dying of tuberculosis because they cannot afford treatment , I feel gloomy .That ‘s why I want to construct this ward”.

The ward was constructed and remained functioning throughout his life. A well know figure: politician, film maker-actor, famous folk singer but who always loved to be known as a social worker’ is not with us today but his services to the Hospital are immortal .

My children don’t require my money why shouldn’t this be used for a more noble cause!

A few years back, a dignified person came to our Hospital and showed his inclination to donate some money to the Hospital. Upon inquiry regarding his objective, he said : “People want to leave assets for their children. .Alhamdolillah (Thanks to Almighty ) both of my sons are working as radiologists in USA and earning handsome salaries . They don’t require my money’ and I want to leave a notable asset for them .So I will be pleased if this money is utilized for a noble cause like construction of a ward in this Hospital”. In this way, a complete surgical unit was constructed with Operating Theatre, rooms and two wards for general patients. The philanthropist not only constructed the Ward but also agreed to bear the running expenditure of the Ward .A reasonable amount is being deposited into the account under head of the said Ward every year, sufficient to meet the expenses of maintenance of the ward.

What a wonderful asset for his sons he has planned!

I am not Begum Sahiba

A philanthropist visits a ward at Gulab Devi Chest Hospital and asks the doctor incharge if his patients need anything. The story is not new for the doctor who is used to meet such donors. He introduces the lady to his colleagues, “Begum Sahiba (Madam) wants to help our poor patients. Can you think of some urgent requirement”? The lady immediately responds, “Don’t call me Begum Sahiba, doctor. I served as an Aya in a school throughout my life and this petty amount which I want to donate, is the money I got on my retirement. I want to donate this money which is very little but is the lifetime achievement for me.”

“No words for your greatness lady! You are indeed Begum Sahiba for me”, says the doctor.

Mian, Don’t Do it!

The Chairman of the Managing Committee of Gulab Devi Chest Hospital, inspired by the healthcare arrangements abroad, wants to implement the idea of dining halls so that the patients may be served meals collectively. He arranges for construction of halls and the practice starts. Soon he comes to know that the patients have refused to take meals in dining halls. Annoyed, he himself visits the dining halls when the meal is served. He wants to know the reason for this attitude. An elderly patient comes to him and says: “Mian (Sir), don’t do it”. Why? The Chairman is eager to know. With tears in his eyes, the old man explains: “The meal which is served here is sufficient for our kids also: they cannot avail this if we are served meal in dining halls”.

Impressed with his argument, the Chairman reversed his decision.

Gulab Devi Post Graduate Medical Institute

In order to achieve our objectives, work on establishment of a Postgraduate Medical Institute was started. The feasibility studies in 2003 ultimately bore fruit and in 2004, Gulab Devi Postgraduate Medical Institute was established and it gained affiliation with University of Health Sciences, Lahore. As a consequence, Diploma in Cardiology and Diploma in Tuberculosis & Chest Diseases were started for postgraduate Doctors in 2005-2006 session. These diploma courses are presently in progress successfully. Research publications by postgraduate doctors is also in the offing and each postgraduate doctor has been allocated his field of research and specific topic allotments is under process.

Gulab Devi Hospital has been given full accreditation for training in FCPS Cardiology by the College of physicians & Surgeons Pakistan. It has also been accredited for training in Pulmonology for Diploma of College of Physician & Surgeons of Pakistan. The Department of Cardiology at Gulab Devi Hospital has been recognized for MRCP training by the Royal College of Physicians in England.

Todate, two doctors have already received their FCPS degree from College of Physicians & Surgeons Pakistan. Six postgraduate doctors have further been enrolled for FCPS Cardiology Training in cardiology Department. Six candidates have already passed their Part-I for MRCP in Cardiology Department. Five postgraduate doctors have completed their part-II MRCP.

In pursuance of the directive of the Governor Punjab, the University of Heath Sciences, Lahore has speeded up its efforts for development of highly trained paramedical personnel. First closed heart operation in the Hospital was performed in 1984 when closed mitral valve commissurotomy was performed in a case of severe mitral stenosis. A full fledged Cardiac Unit was established in 1987, comprising of both cardiology and Cardiac Surgery. Cardiac Operation Theater, ICU , laboratory and wards were constructed and equipped. First open heart operation in this Unit was performed in 1987 . At present, all forms of closed and open heart surgery are performed in this Unit including valvotomies, valve replacement, valve repairs, correction of congenital heart defects, CABG etc . Most of these operations require expensive disposables but many of the patients are so poor that these have to be carried out almost free.

The Department is staffed by two full time Cardiothoracic surgeons , trained registrars and medical officers ,trained nurses and para-medical staff.

Routine ETT is done in Cardiology Department throughout the week.

 Angiography lab

There are two angiography labs available in the Cardiology Department where all types of diagnostic as well as interventional work is carried out under supervision of most experienced cardiologist in the city.

The first lab started its functions in 1996; the second lab, which is equipped with latest digital angiography machine, has started functioning in 2001.Two Angiography Labs are under construction and will be completed within six months.

 Coronary Angiography

It is a routine investigation to localize the site and extent of the narrowing of heart vessels. This helps to decide about the various modes of treatment applicable to certain patients such as angioplasty / stent insertion or bypass grafting.

Coronary angioplasty / Stent Insertion

Coronary angioplasty is the method of opening the narrowed blood vessel of the heart with the help of balloon catheter through artery of the leg without major surgery .To maintain the patency of vessel permanently , a spring like device called stent is placed at the site of previous narrowing of the vessel.
This procedure is routinely carried out at the Hospital under the supervision of well trained interventional cardiologists.

Balloon Valvuloplasties

The tight valves of the heart are opened with balloons.
Rheumatic heart disease is a major affliction of heart that deforms the heart valves and also narrows them. PTMC is a procedure by which these valves are opened by means of a balloon catheter, obviating the need of major surgery. Ours is the centre in which maximum number of these procedures are performed in the city.

Chest Surgical And Emergency Unit

Emergency Department

Adjacent to the Outdoor Department, Emergency Department stands to serve patients presenting in serious condition. The Department consists of an Emergency ward, three procedure rooms, 2 male & 2 female wards. All the patients coming in emergency ward are provided with management of acute illness after which those requiring admission are admitted in the male & female wards comprising of 48 beds. Minor procedures, like pleural & ascetic taps, tube thoracostomies, lumber punctures and incision drainage are frequently done in the procedure rooms. Patients requiring thoracic or orthopedic surgery are shifted to the Surgical wards situated upstairs. Patients requiring vigilant care for their medical and chest problems are kept in Emergency or referred to Chest I.C.U.

The Department provides services to 40 patients per day, on an average. The Department is looked after by Casualty Medical Officer, one Chest Physician and one Thoracic Surgeon on all the working days and round the clock in case of emergency.

Orthopedic Department

An important sequel of Tuberculosis is tuberculosis of bones and joints. This has got a greater morbidity and mortality as compared to other types of Tuberculosis. Services in this regard are being provided to all patients on both outdoor and indoor basis .

An orthopedic surgeon is looking after this Department with the help of a medical officer. Both outdoor and indoor management, along with surgical remedies like spinal implants, are frequently done here along with minor surgical procedures like drainage of psoas and para-spinal abscesses. All the treatment facilities are provided free of charge to deserving patients.

Chest Surgical Unit Department

Thoracic Surgery is being done in this Hospital since 1948. Before the advent of chemotherapy, surgery was thought to have an important role in treatment of tuberculosis. After the discovery of effective chemotherapeutic agents, role of surgery was curtailed to the management of various complications arising from tuberculosis. In addition to managing different difficult situations like empyema or pneumothorax and MDR TB, Thoracic surgery is involved in diagnosis as well as treatment of a long list of diseases including Lung Cancer or Destroyed Lung, hydatis disease and COPD.

At present two thoracic Surgery units are functioning supervised by thoracic surgeons .A variety of procedures are being performed safely in the operating rooms while a satisfactory post operative care is also available. New inclusion in the Department is Video-Assisted Thoracic Surgery which is quite popular in the current scenario. This minimal access surgery is solving many diagnostic dilemma as well as providing  therapeutic remedies to many surgical situations. VATS is being done  in addition to all the major surgical interventions done in the field of thoracic surgery. The data of this unit provides a good view of significant success rate with very little morbidity and mortality

TB Wards

Tuberculosis is still rampant all over Pakistan with a case load of 231 per 100,000 population .Poor TB patients require a centre of excellence where all diagnostic and treatment facilities exist. A TB patient if left untreated can spread this disease to 15 other healthy individuals ,thus treating a TB patient is the most rewarding community service. Gulab Devi Hospital Lahore is fighting against TB since its inception in 1934.Today’s most efficient tool to fight and control TB is DOTS(Directly Observed Treatment Short course).

Since September 2006, the Hospital has implemented DOTS, and results are most rewarding. However, need for a hospital cannot be denied for complicated and drug resistant cases as well as emergencies.

Multi Drug Resistant TB is an emerging threat and requires a lot of resources and trained manpower. But fulfilling its commitment to TB patients, GulabDeviHospital, Lahore is providing all diagnostic facilities and treatment to all cases of Multi Drug Resistant TB.A special ward is allocated for MDR TB and highest number of such patients are getting benefit free of charge.

In-patients Department

In-patients Department includes TB wards, Oncology ward, Asthma and other Non-TB chest wards, Chest Surgery Unit and Cardiac Complex.

In 1947, Hospital started with only 50 beds, but, at present, Hospital is serving 1500 in patients. These patients are suffering from  chest diseases including Tuberculosis and cardiac ailments. Complicated cases of TB , diagnostic problems and Multi Drug Resistant Tuberculosis  require indoor care of patients and  occupy most of our indoor beds. All required investigations and treatment is provided free of charge. Treatment of one patient of Multi Drug Resistant TB approximately costs Rs 100,000 to Rs 150,000. but with the help of philanthropistsHospital is providing this treatment to all patients free of charge for a duration of almost 2 years. All other cases of TB( drug sensitive) are treated through DOTS(Directly Observed Treatment Short course) for a duration of 6 months .

All indoor patients are provided  good food at breakfast, lunch and dinner without any cost.

It is not the body aches and pains or physical stresses only but the suffering is much more. There are mental and socioeconomic strains also. There are men who are the only bread-earners for their families and are lying helpless in Hospital beds. They are always worried about their children and parents back home who may not have anything to eat or wear. And there are women who have been divorced only because they have tuberculosis. They are frightened for they do not know what lies in future for them or their kids!

Quaid-e-Azam’s visit of Gulab Devi in 1947

words of quaid-e-azam

“ I visited the Gulab Devi Hospital which is now tending to the refugees on 6th November, 1947. Those who are incharge of it viz Doctors, Nurses and other are doing excellent work and deserve our thanks for their selfless devotion to this humanitarian work”.
By Quaid –e- Azam Muhammad Ali Jinnah on 06 November,1947

Oh God! Is That The State Of Affairs?

The Medical Superintendent takes a round of the Hospital every day. He knows that every patient of Gulab Devi Chest Hospital gets an egg in his breakfast but was surprised to see six or seven eggs in the bedside locker of a patient. Thinking this might be a pilferage, he asked the patient how he had got more than one egg. The patient replied ,”Sir, my children have never taken egg throughout their lives. Tomorrow is Sunday and my family is expected to visit me .I have saved these eggs during the whole week so that they may also taste egg.”

My God! Is that the state of affairs? The Medical Superintendent couldn’t believe it.

Out-Patients Department

The Hospital has a very busy Out-Patients Department. On average, 1000 patients visit OPD every day from all over the country. They have respiratory symptoms and come either for diagnosis of their disease or for treatment.

Emergency patients are immediately referred for admission and treatment. All kinds of patients of TB (TB of lungs and other parts of body) and other chest diseases are diagnosed, treated and managed accordingly. Routine cases who need admission are admitted and others prescribed treatment. The new achievement is the establishment of DOTS Unit, where TB patients are diagnosed and treated free of cost according to WHO guidelines. Follow up of patients, discharged from wards is also done in OPD where patients are provided drugs free of charge for a total period of 06 months in accordance with WHO guidelines.

We want to give best possible treatment to every needy patient who enters this Hospital. With the help of Government grants and donations from private sector, a lot of work is being done. Many kind-hearted donors come and donate money to the Hospital, some construct buildings for wards, some buy equipments and some help with donations in kind. But still patients are suffering for want of investigations or drugs. We have to improve the quality and quantity of treatment. Let not the lack of resources come in our way. Help our patients by giving any amount, in cash or kind, or even with kind soft words by visiting them.
